Nurse: Case Management (Thousand Oaks, CA)

Job Description:

The Case Management Manager (CMM) is a registered nurse with advanced training and experience in Case Management. This position reports to the Director of Case Management. The Case Management Manager is responsible and accountable for monitoring the case management process throughout the hospital utilizing the established case management process and tools, established policies and procedures, established professional standards, and administrative hospital-wide policies. The Case Management Manager proactively coordinates and implements all aspects of the Case Management program.

Top "must-have" skills for this job Minimum 4 years of acute care experience (medical/surgical, ICU or ED preferred) Min 3-5 years experience as a case manager BSN; current RN license
